Dear Providers:
Alere/Optum provided multiple programs and services for our FL Staywell MMA (Medicaid) members. Starting August 1, 2017, Staywell will handle the programs listed below instead of Alere/Optum. The programs include:
- Disease Management: This program provides education, guidance, and support to individuals with a chronic health disease such as asthma, diabetes, coronary artery disease, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) and hypertension.
- First Year of Life: This program supports the parent/caregiver during the infant’s first year of life. This program focuses on areas that are important to the infant’s first year such as health; nutrition; immunizations; growth and development; home safety and promotion of recommended scheduled provider visits.
- Tobacco Cessation: This program offers a Health Coach to guide and support those who want to stop smoking.
- Weight Management: This program offers a Health Coach to educate, guide, and support those who want to lose weight.
- Healthy Rewards Gift Card Fulfillment and Wellness Portal: This program rewards members that complete certain health visits. Rewards are loaded onto a debit card, gift card, or e-gift card. Members can log on to the portal and access this program at www.wellcare.com/Florida.
Alere/Optum will continue to provide services for our OB/Maternity program.
