Looking for your plan documents?

Follow these three easy steps to find documents for your specific Medicare Prescription Drug Plan.

1. Visit wellcare.com/find-your-plan      
2. Choose your plan type: Select Prescription Drug Plan (PDP)
3. Select your state and search: Choose the state where you live, then select Search.
4. Choose your plan

Select your plan to view and download important documents, including your Evidence of Coverage (EOC), Annual Notice of Changes (ANOC), Summary of Benefits, and other member materials.
