24-339m Complete the Special Needs Plan Model of Care Training

Participating providers must complete this CMS-required training annually

The Centers for Medicare & Medicaid Services model of care training required for Wellcare Medicare Advantage (MA) providers is available online. This applies to Dual Eligible Special Needs Plan (D-SNP) members who are eligible for both Medicare and Medicaid, and our Chronic Condition Special Needs Plan
(C-SNP) members.

The training is designed to help you better understand Wellcare’s approach to the delivery of care for Special Needs Plan members. Participating providers must complete this training by December 31, 2024, as required by the Centers for Medicare & Medicaid Services.

How to access the training to comply

Wellcare requests physicians and other providers who treat Special Needs Plan members to submit a voluntary attestation after completion of the model of care training for our records. The attestation will be made available after completing the model of care training.

To access the training, select the Model of Care annual training recording link available on the model of care training page.

Please note that completion of this training is auditable by the Centers for Medicare and Medicaid Services.

On April 22, 2024, UnitedHealth Group issued a press release, providing an update on the Change Healthcare cybersecurity incident that occurred on Feb. 21, 2024. Given the size of the data impacted, the investigation to determine whose data is impacted is expected to take several months. UnitedHealth Group believes this situation will impact “a substantial proportion of people in America” and is offering immediate credit monitoring and identity protection services, as well as a dedicated contact center to address questions.
