Initiating care and providing ongoing treatment for medical and/or behavioral health concerns is critical in improving a member's health and well-being. Wellcare offers Case and Disease Management Programs developed to address several medical and behavioral health conditions.
How to Refer Wellcare Patients to Case and Disease Management
If you would like to refer your Wellcare patients to either or both of these programs, please call the Case and Disease Management Referral Line at 1-866-635-7045, Monday-Friday from 8 a.m. to 5 p.m. ET.
Clinical Practice Guidelines
Clinical Practice Guidelines (CPGs) are best practice recommendations based on available clinical outcome and scientific evidence. Wellcare CPGs reference expert professional and clinical society recommendations, ensuring that the guidelines contain the highest level of evidence-based content. CPGs are also used to guide efforts to improve the quality of care in our membership.
Quick Reference Guide
Wellcare's Quick Reference Guides (QRGs) contain key contact information and authorization guidelines. The QRGs are updated regularly, so please check back to ensure you have the most current version. QRGs can be found on your state's Medicare Overview page.
