Thank you for being a Prescription Drug Plan Member!

Access your Summary of Benefits, Evidence of Coverage (EOC), and other important plan documents on the Plan Overview page.

Start with the Find My Plan tool to locate your plan information:

Step 1: Start at www.wellcare.com/PDP and use the drop down to select your state.
Step 2: Under "Plan Type", select “Prescription Drug Plan,” enter your zip code, then click “Search.” The plans available for that zip code will populate.
Step 3: Find your plan on the list and click “Go to My Plan Details.” 

You can view your Comprehensive Formulary, a list of prescription drugs covered by your plan, or search for specific drug via the Drug Search Tool on our Drug List (Formulary) and Other Documents page, located under the Pharmacy tab to the right of the page.

On April 22, 2024, UnitedHealth Group issued a press release, providing an update on the Change Healthcare cybersecurity incident that occurred on Feb. 21, 2024. Given the size of the data impacted, the investigation to determine whose data is impacted is expected to take several months. UnitedHealth Group believes this situation will impact “a substantial proportion of people in America” and is offering immediate credit monitoring and identity protection services, as well as a dedicated contact center to address questions.
