Effective June 1, 2026, Wellcare is partnering with Monogram Health in Texas to provide additional support to our members with chronic kidney disease (CKD), stages 3b–5, and end-stage kidney disease (ESKD). Monogram is the leading multispecialty provider of in-home, evidence-based care for patients with multiple chronic conditions.
All eligible Wellcare members with CKD, stages 3b–5, and ESKD can benefit from Monogram’s value-based, in-home, multispecialty treatment and care for patients with multiple chronic conditions. Monogram takes a comprehensive and personalized approach to a person’s health, evaluating the whole person and treating all chronic conditions present alongside CKD and ESKD, such as diabetes, hypertension, heart failure, depression, Chronic Obstructive Pulmonary Disease (COPD), and other metabolic disorders.
Monogram can work individually or alongside providers to treat and care for Wellcare members. Monogram employs a robust and integrated clinical team, leveraging specialists across multiple disciplines, including nephrology, cardiology, endocrinology, pulmonology, behavioral health, and palliative/end of life care to:
- Diagnose and treat all present and prevalent chronic conditions.
- Review and prescribe medication.
- Provide guidance, education, and counselling on a patient’s healthcare options.
- Assist with daily needs, such as access to food/healthy eating, obtaining transportation, financial assistance, and more.
Monogram is available 24 hours a day, 7 days a week, and on holidays to support and treat patients at home. Think of Monogram as an extension of your practice.
