You have options.

Are you a member? Would you like to request a printed provider/pharmacy directory? We can help. Call our Member Services number. You can find it on the back of your Member ID card. You can also self-service. First, log into our secure website. Then select “Order Member Materials.”

Not a member? No problem. You can also request a printed provider/pharmacy directory if you are not a member. Call 1-866 527-0057 (TTY 711). For Ohana Health Plan, please call 1-866-907- 2058 (TTY 711). Hours of operation are Monday–Friday, 8 a.m. to 8 p.m. Between October 1 and February 14, representatives are available Monday–Sunday, 8 a.m. to 8 p.m.

On April 22, 2024, UnitedHealth Group issued a press release, providing an update on the Change Healthcare cybersecurity incident that occurred on Feb. 21, 2024. Given the size of the data impacted, the investigation to determine whose data is impacted is expected to take several months. UnitedHealth Group believes this situation will impact “a substantial proportion of people in America” and is offering immediate credit monitoring and identity protection services, as well as a dedicated contact center to address questions.
