Special Needs Plan Model of Care MOC Training

August 7, 2023

Dear Provider,

The Centers for Medicare & Medicaid Services (CMS) requires health plans to provide annual education and training on Wellcare’s Model of Care to providers who treat our Special Needs Plan (SNP) members. This applies to our Dual Eligible Special Needs Plan (D-SNP) members, who are eligible for both Medicare and Medicaid, and our Chronic Condition Special Needs Plan (C-SNP) members, who have one or more qualifying conditions.

As stated in the Provider Manual, all providers who treat SNP members must complete Wellcare’s Model of Care training. The annually updated training is available for download and self-study at: Wellcare.com/Providers/Model-of-Care-Training

We appreciate the quality care you provide to our members and your support of our efforts to meet CMS regulations.

For additional information on how to work with our health plan to manage SNP members, please visit your state's provider Overview & Resources page at Wellcare.com/Providers. This site has links to the Provider Manual, Quick Reference Guides, Clinical Practice Guidelines, and more.

Sincerely,

Wellcare Health Plans
