Harmony Health Plan Member Documents

Your Member Handbook explains your plan coverage in detail. It includes important information about your benefits, your rights, and key contact addresses and phone numbers.

Harmony Health Plan does not decide if you qualify for into the Waiver or Nursing Home programs. The Department on Aging or the Department of Human Services, Division of Rehabilitative Services, makes this decision. If one of these Departments decides you qualify, you will be asked to select a health plan.

The State of Illinois mailed you a letter asking you to confirm information on file for your Medicaid coverage. This is the Redetermination process. You must take action.

How to Keep Your Coverage

You got a letter asking you to confirm information about your Medicaid coverage. You may also need to report any changes or updates. You must respond by the date on the letter. You could lose your Medicaid coverage and your Harmony benefits if you don’t. 

The State of Illinois Application for Benefits Eligibility (ABE) website gives you information about your:

  • Benefits, including application status
  • Redetermination status
  • Recent notices
  • Benefit amount

The website also lets you:

  • Submit redeterminations
  • Report changes
  • Request new benefit programs

Want to learn more about how to use ABE Manage My Case? You can visit the Illinois Department of Human Services ABE Customer Support Center. It is at http://www.dhs.state.il.us/page.aspx?item=98456.

Redetermination Submission Information:

Step 1: Go to http://ABE.Illinois.gov 

Step 2: Click the green “Manage My Case” button. Complete the login process or click “Create an ABE” account

Step 3: After logging in, select “Link your account”

  • Enter your date of birth and Social Security Number or ID number. This will link your ABE account to your benefit case information

Step 4: When you link your case to your ABE account, you will be asked to confirm your identity. If successful, you will go right to MMC
