‘Ohana's QUEST Integration program provides a variety of medical services to keep you healthy. As a member, you can choose a primary care provider (PCP) to be your personal doctor.

Getting Services Covered

Find out how to get services covered by ‘Ohana Health Plan. There are no co-pays or co-insurance for this plan, except for cost-sharing as described by your plan.

QUEST Integration Plan Documents

You can request for us to mail you a paper copy of any printed material. Just log into your secure web portal and order a copy. You can also call customer service to request a copy. The material will be provided to you within 5 business days. All at no cost to you.

Your Member Handbook explains your plan coverage in detail. It includes important information about your benefits, your rights, key contact addresses and phone numbers.
