Over-the-Counter (OTC): Get $25 worth of OTC items sent to you each month. (per household) Items are mailed at no cost.
Transition from Skilled Nursing Facility to Private Home Setting: Provides up to $2,500, per lifetime for the child's private home setting if they are in a skilled nursing facility and transitioning to a private home setting within the community
Swimming Lessons (Drowning Prevention): Members under age 21 can receive up to $150 per enrollee with prior authorization (space is limited)
Same-Day-Procedure Meals Allowance: Receive up to $200 per day (maximum $1,000 per year) for trips over 100 miles from home
Free Cell Phone Program: Get a free cell phone from Safelink/TracFone. Includes 350 minutes for talk and unlimited texting
CVS™ Discount Card: Save 20% on thousands of CVS/Pharmacy brand health-related items with the ExtraCare Health Card. Use it in stores or online at www.CVS.com. (applies to regularly priced items of $1 or more)
Housing Assistance: Receive up to $250 per year for housing assistance and up to $75 per quarter to purchase healthy food items
Home Delivered Meals - Post-Facility Discharge (Hospital or Nursing Facility): Members discharged within two weeks from an inpatient facility (Hospital, Skilled Nursing Facility or inpatient Rehabilitation) may be eligible to receive 10 meals per authorized request
Expanded Non-Medical Transportation: Provides transportation for members, as necessary for community integration (up to 2 round-trips per month)
Nutritional Counseling: Assessment, hands-on care, education, and guidance about nutrition for caregivers and members, including initial assessment, intervention and individual, face-to-face with the patient (15 minutes each)
Behavioral Health Services for Caregivers: Education and training for patient self-management by a qualified, non-physician health care professional using a standardized curriculum and 30-minute face-to-face sessions with the patient/caregiver/family
Doula Services: Doula services for members with a goal of improved birth outcomes, reduced pre-term births, and improved prenatal care for members ages 13 to 21
Home Delivered Meals - Disaster Preparedness/Relief: Provides 1 emergency meal kit per year
Computerized Cognitive Behavioral Analysis for Non-Medicaid Caregivers: Including, but not limited to the following: health-focused clinical interview, behavioral observations, psychophysiological monitoring, health-oriented questionnaires, health and behavioral interviews (individual, group, family – with or without the patient)
Newborn Circumcision: Provides for one circumcision per lifetime for newborns up to 28 days old
Home Delivered Meals – General: Qualified members may receive up to 10 meals per authorized request.
Community Connections Help Line: FREE Community Connections Help Line connects you to services such as utility assistance, food banks and transportation in your community
Steps2Success: Helps position your child for success with free services like:
- Reading scholarships for qualified members (Pre-Kindergarten to 12th grade) who want to improve their reading skills (space is limited)
- General Educational Development® (GED®) testing for members age 16 and older who do not have a high school diploma; 1 voucher per year, per member (covers 4 tests)
Tutoring Services: Receive 12 tutoring sessions to aid in removing educational barriers; Up to 2 hours of tutoring time per session
Adaptive Devices – Housing-related: Covers adaptive devices such as jar openers and doorknob grippers to support members living at home (1 item per year)
Benefit Counseling for Caregivers: Provides benefit counseling to caregivers and support staff (3 sessions per year)
Financial Counseling: Members and their families can get 6 financial counseling sessions per year
Healthy-Wellness Coaching: Provides wellness coaching to encourage living a healthier lifestyle (12 visits per year)
Pest Control: Provides pest control for homes of qualified members (up to $500 per year)
Planned Respite: Caregivers of members will receive 200 hours of in-home respite and 10 days out-of-home respite per plan year
Wellness Items: Get education and support for therapeutic items and supplies (up to $200 per year)
HEPA Filter Vacuum Cleaner: Members with asthma may qualify for a vacuum cleaner with a HEPA filter that can trap pollutants and may help with allergies
Carpet Cleaning: Members with asthma may qualify for 2 carpet cleanings per plan year
Hypoallergenic Bedding: Eligible members with asthma can get a hypoallergenic mattress and pillow casings to help with asthma triggers
Healthy Rewards: Rewards members who complete specific preventive health, wellness, and engagement milestones
