You may be eligible enroll in a WellCare Medicare Advantage plan if you:
- Live in our plan's service area
- Are entitled to Medicare Part A
- Are enrolled in Medicare Part B
- Are entitled to Part D coverage if you want to enroll in a Medicare Advantage plan with Prescription Drug Coverage or our Stand-Alone Prescription Drug Coverage plan
- Do not have end-stage renal disease (ESRD).There are limited exceptions, including:
- If you develop ESRD when you are already a member of the plan
- You were a member of a different plan that was terminated
- If you have received a kidney transplant and no longer require a regular course of dialysis to maintain life (you are not medically determined to have ESRD)
- Are suffering from Amyotrophic Lateral Sclerosis (ALS)
- Are a United States citizen or have recently obtained lawful presence in the United States
