Healthy You, Healthy Baby!

The Planning for Healthy Babies® (P4HB®) Program Cares About Your Health.

We provide family planning services to women who do not qualify for other types of Medicaid benefits. We work to reduce the rate of low birth weight and very low birth weight births in Georgia.

There are things you can do to help make sure your baby is born at a healthy weight. Georgia's P4HB® program is here to help. The goal of P4HB® is to help  women be healthy and plan ahead in order to have healthy babies when they are ready to start their families!

Planning for Healthy Babies® is the 1115 Demonstration waiver program in Georgia supported by CMS. It expands the delivery of family planning services to women who:

  • Are 18 through 44 years of age and are uninsured;   
  • Have family income at or below 200 percent of the Federal poverty level (FPL);
  • Are not pregnant but who can become pregnant; and
  • Are not otherwise eligible for Medicaid or PeachCare for Kids®.

Applying is Easy.

Just go online or get an application at your health department or DFCS office. You can also get an application at an FQHC (Federally Qualified Health Center) in your area.

The Georgia Association for Primary Health Care helps people with no insurance or not enough insurance to get primary care.

This will help Planning for Healthy Babies® members find affordable primary care services they might not have access to under their family planning health plan.

P4HB Plan Documents

Your Member Handbook explains your plan coverage in detail. It includes important information about your benefits, your rights, and key contact addresses and phone numbers.

The Mommy and Baby Matters Booklet contains important information for moms-to-be including prenatal, postpartum and newborn care.

The Over the Counter Catalog includes a detailed list of available items from allergy medications to sunblock to vitamins. It also includes easy instructions on how to order.

Earn $20 when you attend 6 or more prenatal visits before the birth of the baby. Just print this form, and complete the information with your health care provider.

The Transportation Notice document includes instructions on how to set up an appointment for transportation to a medical appointment.
