Medicare : Special Needs Plans
Special Needs Plans for Dual Eligible individuals (D-SNPS) are specially designed Medicare Advantage plans that include Prescription Drug Coverage and other benefits for people who have Medicare and Medicaid. Depending on your level of Medicaid, WellCare may have a D-SNP that meets your needs.
These plans are available to anyone who has both Medical Assistance from the State and Medicare.
Access Plan:
To be eligible for this plan, potential enrollees must be QMB-only or QMB Plus or qualify as any other dual-eligible for whom the state holds harmless for Part A and B cost sharing.
NY Advocate Complete Plan:
To be eligible for this plan, potential enrollees must qualify for Medicare Parts A and B, full Medicaid benefits, and be eligible for a nursing home level of care but be capable of returning to or remaining in the home and community without jeopardy to health and safety.
NY Liberty Plan:
To be eligible for this plan, potential enrollees must qualify for Medicare Parts A and B, and qualify for full Medicaid benefits as a QMB Plus or Full Benefit Dual Eligible.
Select Plan: *Only available in Florida
To be eligible for this plan, potential enrollees must be determined to be eligible for full or partial medical assistance from their state Medicaid agency and includes all categories of Medicaid eligibility.
'Ohana Liberty (HI): *Only Available in Honolulu
To be eligible for this plan, potential enrollees must be eligible for Medicare Parts A and B, qualify for full Medicaid benefits (QMB Plus, Full Benefit Dual, or SLMB Plus) and be a enrolled in the state's Medicaid managed care program for aged, blind and disabled (QEXA).
