How To Enroll

1. Enroll online - View our plans and complete your application online.You can enroll online; or

2. Enroll by mail - download, print, fill out our Enrollment Form and mail it to WellCare by using the address within the form; or

3. Enroll by phone - our representatives can enroll you right over the phone. Contact us to enroll; or

4. Medicare beneficiaries may enroll in WellCare through the CMS Medicare Online Enrollment Center located at http://www.medicare.gov/.
