Medicaid : New York Healthy Choice Eligibility Chart

If you or your children currently have Medicaid, you can join WellCare’s Healthy Choice plan now. If you do not have Medicaid, but are eligible for it, you are also eligible for Healthy Choice. In some counties, people with Medicaid must join a health care plan.  Such counties operate a mandatory managed care program.  Other counties allow Medicaid consumers to choose whether they want to join managed care.  These counties operate a voluntary Medicaid managed care programs.  Both programs, though allow for some people to keep getting care through regular Medicaid.

The following list of counties shows which areas are mandatory in WellCare's service area:
