To be eligible for WellCare’s Family Health Plus plan, you must live in WellCare’s New York service area, which includes the counties of Kings (Brooklyn), Bronx, Queens, and New York (Manhattan). For other eligibility requirements, please refer to the charts below.

 Adults Who:

• Are between the ages of 19 and 64 years old
• Are not eligible for Medicaid
• Do not have health insurance
