Children Who Are:
- Under 19 years old
- Not eligible for Medicaid
- Have little or no health insurance
- Residents of New York State
May Qualify:

| 4 | $7,350 or less |
| 5 | $8,597 or less |
| 6 | $9,844 or less |
| 7 | $11,090 or less |
| 8 | $12,337 or less |
* Call to speak with a representative if you have more than 8 members in your family.
