As of December 31, 2006, Harmony Health Plan of Indiana ceased operation as a Medicaid Managed Care Organization (MCO) with the Hoosier Healthwise program. Claims for services rendered to Harmony members prior to 12/31/06 need to be submitted within contractual timeframes. For questions regarding timely filing timeframes, you may contact Harmony Provider Services at 1-866-334-7713. Harmony will continue to accept paper or electronic submitted claims. Paper claims may be filed by mailing to P.O. Box 31372, Tampa, Florida, 33631-3372. Providers wishing to submit electronically may do so using payer ID number 14163.
