Have questions about Staywell? We want to make it easy for you to find answers. Below are some commonly asked questions. If you have a question that isn’t listed here, please call a call a Benefit Specialist, toll free at 1-800-963-2456 between 8 am-7 pm Monday Friday Eastern Time.

1) How Do I Know if I Am Eligible For Medicaid?

The Florida Department of Children and Families (DCF) determines all other Medicaid eligibility including programs for children and families; blind and disabled; and institutional care. If you would like to learn more about qualifying for Florida’s Medicaid program, please call 1-888-367-6554 (TTY/TDD 1-800-653-9803) to speak with a Medicaid Options Representative, or 1-866-454-3959 (TTY/TDD 1-866-467-4970) to speak with a Choice Counselor.

2) How Do I Know if I Am Eligible For Staywell?

To obtain Staywell, you must live in the following service areas:

Brevard	Lee	Pasco
Broward	Manatee	Pinellas
Dade	Orange	Polk
Hernando	Osceola	Sarasota
Hillsborough	Palm Beach	Seminole

3) Do I Have A Choice Between Medicaid And Staywell?

Most Medicaid recipients are required to get services through managed care.
After you are approved for Medicaid, you will be sent information on managed care providers like Staywell. You must choose your plan within 60 days. If you do not, the state will choose a plan for you.

4) What Does Staywell Offer?

Staywell offers all the benefits of Medicaid:

• Adult vision services including unlimited eye exams and eyeglasses,
  if medically necessary
• Adult hearing services to include one hearing aid every three years, if medically necessary

PLUS

• Adult dental benefits such as unlimited fillings, periodontic deep cleanings, X-rays, annual exam, two cleanings per year and extractions 
• Up to $25 per month for selected personal care items for every household

5) How Do I Enroll in Staywell?

Staywell serves children and adults eligible to participate in Florida’s Medicaid program. To learn more about Staywell, call 1-800-963-2456 to speak with a Benefit Specialist today!

6) If I Already Have Medicaid, How Do I Switch to Staywell?

• Step 1: Call a Benefit Specialist, toll-free at 1-800-963-2456
• Step 2: When prompted, choose option 3 to speak with a Benefit Specialist

You need to listen to all the requirements BEFORE selecting options.
