1) How Do I Know if I Am Eligible For Medicaid?

The Florida Department of Children and Families (DCF) determines all other Medicaid eligibility including programs for children and families; blind and disabled; and institutional care. 3) Do I Have a Choice Between Medicaid And HealthEase?

Most Medicaid recipients are required to get services through managed care. After you are approved for Medicaid, you will be sent information on managed care providers like HealthEase. You must choose your plan within 60 days. If you do not, the state will choose a plan for you.

4) What Does HealthEase Offer?

HealthEase offers all the benefits of Medicaid: 

• adult vision services including unlimited eye exams and eyeglasses,
  if medically necessary
• adult hearing services to include one hearing aid every three years, if medically necessary

PLUS

• adult dental benefits such as unlimited fillings, periodontic deep cleanings, X-rays, annual exam, two cleanings per year and extractions
• up to $25 per month for selected personal care items for every household

5) How Do I Enroll in HealthEase?

6) If I Already Have Medicaid, How Do I Switch to HealthEase?

• Step 1: Call a Benefit Specialist, toll-free at 1-877-431-8862
• Step 2: When prompted, choose option 3 to speak with a Benefit Specialist
