1) How do I know if I am eligible for Medicaid?

The Florida Department of Children and Families (DCF) determines all other Medicaid eligibility, including programs for children and families, blind and disabled, and institutional care. If you would like to learn more about qualifying for Florida's Medicaid program, please call 1-888-367-6554 (TDD: 1-800-653-9803).

 

2) How do I know if I am eligible for HealthEase?

To obtain HealthEase, you must live in the following service areas:

Brevard

Manatee

Broward

Marion

Calhoun

Martin

Citrus

Orange

Dade

Osceola

Duval

Palm Beach

Gadsden

Pasco

Highlands

Pinellas

Hillsborough

3) Do I have a choice between Medicaid and HealthEase?

Most Medicaid recipients are required to get services through managed care. After you are approved for Medicaid, you will be sent information on managed care providers like HealthEase. You must choose your plan within 60 days. If you do not, the State will choose a plan for you.

 

4) What does HealthEase offer?

HealthEase offers all the benefits of Medicaid:  
  • Adult hearing and vision (medically necessary services only)
  • Kids hearing and vision, plus well-child checkups
  • Doctor and hospital visits
  • Inpatient and outpatient hospital services
  • Emergency room services
  • Lab and radiology services
  • No-cost prescriptions
  • Behavioral health care and substance abuse treatment
  • Therapy services—occupational, physical, respiratory and speech
  • Chiropractic and podiatry services
  • Family-planning services and supplies
 

With HealthEase:

  • You do not have any co-payments. 
  • Personal Health Advisor, our 24-hour nurse helpline, is always available to you. 
  • If you are pregnant, you can take part in our Prenatal Rewards Program. 
  • You may be eligible for case/disease management services.
  • Plus, you can choose your doctors, hospitals and pharmacies from our large network of providers.
